Some Ideas to think about
 
let me give you a diagram:
O -T Bot
X - CT BOT
:| Hostage

lets say you have this, a CT bot on one side and a T bot on the other with a hostage in the middle... this already doesn't look good...
X :| O

lets say the T bot opens fire on the CT bot, he will just own a hostage, as tho it is just a wall he is shooting thru resulting in this...
X --- 8o --- O

and finally this

this might be solved the same way like avoiding TK can be implemented, just by doing tracelines and watching which entitity is hit ... and often you dont even have to have additional code :)

is this just a fun thread, or a thread pointing out difficulties this bot is having problems with ?

Hmm... not easy to categorize. At a first glance, I thought it was more like a bit of "bot philosophy"... however, you posted it in "ideas". This confused me a bit because strictly speaking you are not saying "Hey, I got an idea - let's do this and that". You are actually describing difficulties the bot can have, just like you posted above. This would point towards the "bug reports" section, but then again, it's not really a bug - more a limitation... oh well, I guess it's ok to leave the thread here... 9_9
